That is because those items are not really form fields. Reading line by line or word by word I found them understandable. The form appears to expect that those item will be completed with pen and paper, not uncommon for release forms. Which version of Adobe reader are you using. We found some significant problems with forms with Reader 8 and JAWS 9 and 10.

Hmm, the only odd thing I noticed (using Jaws 10, Build 512) was that
the labels for the signature boxes at the bottom were all read together:

"Signature of StudentDateSignificant Other*Date"

The main form itself behaved quite well.
