because of this reason, I like to build sites so that the page title appears ABOVE the navigation. This means that the page title is read out, and the user will know where they are on the site. They are then offered the "skipnav" link. If the user is on a page that they don't want to be on, they can listen to the navigation and move to another page, or if they are happy that they are on the correct page, they can skip the navigation and go straight to the content.

Although I realise this isn't always possible if you're trying to take an existing site as this may affect the design and may upset clients/designers.
