I have tested and used Wufoo.com, SurveyMonkey.com and Google Docs Forms. Question types usually not accessible are Likert Scales. All three services are accessible to end users filling out the form. An accessible method of creating the form with these services may not be possible.
I'd like to hear back from the group which services have the most accessible Likert Scale question types?
Has anyone used MachForms and customized the forms to be accessible? It's a PHP script that's pretty cheap if you host it.

Hi all
First post here in a long long time.. so be gentle
I need to send out an online survey to about 2000 people with various
disabilities. The budget does not allow for a custom survey build.
Which online survey tool would people recommend.
I have heard, anecdotally, that SurveyGizmo isn't that good with AT
despite all their promotional claims.
