Yes the attributes are always spoken using the system default. .

I browsed a few pages that have correct lang attributes (for example http://fr.wikipedia.org/wiki/Wikip%C3%A9dia:Accueil_principal) in NVDA. I was surprised to discover that, while text is pronounced correctly, alt attributes are voiced in the default - and therefore incomprehensible - English voice. Is this common behaviour among screen readers?
