The old SM was terrible but in the last few years they have become
exceptionally good. A downside to SM: you cannot buy an account and create
multiple users. So, it's not ideal for certain environments where you have
more than one person on a team who is creating and distributing surveys.
But for shear accessibility, it does very very well. I believe all of its
(fairly limited) question types are accessible.
