we've been using survey gizmo for awhile, it has some good text only/compliant options (we do have a paid account)

In our testing, Survey Monkey is the most accessible. There are things you
can do to make Survey Gizmo quite accessible. Error reporting was a problem
in some browser/screen reader combinations (errors where not focused and
were difficult to locate on the page). Survey Monkey was also the only one
where the survey creation process could be achieved relatively easily using
a screen reader. On the large/enterprise commercial end, it seems like
Qualtrics does a better job than most.

Shouldn't you be using a survey form to ask this question...??? .;-)

I prefer Survey Gizmo primarily because they were the first of the survey
app folks who even knew was accessibility was.

I can remember when Survey Monkey used images for all their actionable buttons ( "next", "submit", etc.) with no alt attribute on them.

The old SM was terrible but in the last few years they have become
exceptionally good. A downside to SM: you cannot buy an account and create
multiple users. So, it's not ideal for certain environments where you have
more than one person on a team who is creating and distributing surveys.
But for shear accessibility, it does very very well. I believe all of its
(fairly limited) question types are accessible.

I know nothing about the extent or accessibility of the creation of
these, and have never seen more than just very simple survey
questions, but basic surveys created with Twiggs I have seen have been
very accessible.
