- axesPDF by xyMedia (still in beta, beta freely available) makes it substantially easier to create accessible PDF from within Microsoft Word (2007, 2010, and probably also 2013, but I haven't checked out the latter myself); cf. http://www.axespdf.com

Please note that some basic implementation in support of accessible document creation has been available for both Microsoft Office (2007 and onward) and Adobe InDesign (CS 5.5 and onward), as well as for OpenOffice/LibreOffice, for a while. For most users though these programs remain a challenge due to less than perfect user interface and sometimes serious technical limitations if not bugs. The add-ons listed above fill most of the severe gaps (both in user interface and regarding technical issues), and turn painful work into productive work, yielding much better results than otherwise feasible.
