Can't get JAWS or NVDA to read through PDF

Hi there,
I created a PDF form and I cant get JAWS or NVDA to read continuously through the PDF. I have all the Heading Tags in the correct order, use lists normally, and have form labels. For the life of me I cant figure out what Im doing wrong. The PDF is set up exactly the way it should be, and it is the same as all the other documents Ive ever done, aside from the fact that this one has form elements.
The form elements dont start until page 5. At first, I thought the screen readers were having issues with the Grouping element Sect, so I eliminated those and let everything sit in a standard weak structure. Then I changed the page numbers from roman lower case for the cover and TOC page and just ran everything with numbers.
Ive used pdfGOHTML and it comes up looking spectacular. I created Accessible text and it looks great. I cant get JAWS or NVDA to read straight through or identify headings. What can I do to save the document and get it to work?
