W3C WAI has a resource on "Strategic Planning for Web Accessibility: Guidance for Developing a Plan for Your Organization or Project" at:
<http://www.w3.org/WAI/impl/Overview>
It is currently a robust draft. We're working on a project to provide a dynamic guide for planning and managing web accessibility, that will include much of this information and more. So we particularly welcome feedback at this time. The main place for feedback is the publicly archived list:
