We at the Ontario provincial government (Canada) are being *subjected* to
similar forms for feedback on various initiatives and I *wish* they were
only 20 questions long. What with the multiple radio buttons (similar to
Tim's), the pages/forms are very, very long.

What about having the options in a Select list? The options (Agree,
Disagree, etc) would be very clear to all and the size of the form (and
possibly the coding) would be much reduced.
