Hello Courtney,
I do a radio button with number options (e.g., 1, 2, 3, 4, 5) and just tell
the user in the question that 1 is low and 5 is high, or whatever the scale
is. Don't use the default scales unless you know they work. Just make your
own. To get the radio buttons you do 5 choices where the user can only
select one.
I use both Microsoft forms and Google Forms, but I've also seen it done on
Qualtrics.
