This is somewhat of a self-serving addition to your list, but WebAIM's
Guide to Web Accessibility Techniques and Concepts, in CD-ROM format
(and in the near future in a downloadable format), should be added to
the list. Although technically it isn't a book in print format, it is
built like a book, with sections and chapters, plus it contains video,
simulations and a few extras. It contains more "pages" than would be
feasible to print in a single book, and is one of the most comprehensive
resources out there.
