Hi Jennifer,
I have used these menus from Project Seven:
http://www.projectseven.com/products/menusystems/pmm/index.htm
They are not free, but they are easy to implement, offer tech support,
and seem to meet all the criteria you cited above. I'm sure on this
topic you will get a gazillion different responses! Good luck.
