Since this is done server side, before the browser sees it, it is
essentially the same as if it were actually part of the page.

Perhaps include a bit of content before the skip to content links/nav menu.
Even if it's just an H1 with the page title...

As an aside, on another list I am on discussing a similar issue, one of the
posters sent this:

I called a student who attends our school to talk about
this issue. His expertise is first hand. That is, he
is blind and uses JAWS.

He says the he prefers the menu at the top. It wasn't
the answer I expected.

He does, however, make three important points...
1. any menu that is repeated on every
page should be as short as possible,
2. there should be no "roll-overs" or
other "motion" on a page (this causes
JAWS to re-read the entire page every
time this happens),
3. there should be no images for links
if at all possible since it reads him
the location of the image file and is
often confusing.
