Subject: Accessible PDF forms vendor

I have a request for a vendor who produces/creates accessible PDF forms.
Does anyone know of a vendor who would work with a client to create an
accessible PDF form? Not that it matters too much, but the person
wanting to hire is located in Sacramento.
