I'm wondering if anyone can recommend a forms generation software
package that produces accessible forms?
I know about the Accessify tool:

I need something self contained that prompts forms creators for all
the information needed to generate accessible forms.

John,
I should have specified that the software package that will generate
accessible forms be usable by mere mortals:-) I can't distribute
AForm and expect that anyone on campus will have the foggiest idea of
how to install and implement it except a very few support people.

There are a number of elearning course content conversion tools which
convert
Word documents into HTML.
Some of the tools convert specially structured tables in Word into
fairly accessible
HTML forms, for example YAWC Online (our product - www.yawconline.com),
and courseGenie (www.horizonwimba.com).
Where you put the HTML pages after they are created is up to the author.
However, the background processing of the forms is a different matter.
It must still be developed in some non-generic way, I think.
Forms generated with elearning tools generally are self-assessments, where
the answers are displayed using embedded JavaScript, or are intended for
use in
SCORM-conformant VLEs/LMSs such as Moodle, Blackboard, etc.
