Subject: Ability to add explanatory text and/or link to a Survey Monkey survey (does anyone with S.M. experience mind answering a few questions on or off-list)?

Hi yea wise ones.

As part of my research (the results of which will be presented at CSUN)
I need to post a 10 question survey regarding browsing habits.
I believed that Survey Monkey had the best accessibility, so I created
the survey through there.

I have put a preliminary copy of the survey online, but have a few
minor issues that I do not see the solutions to yet (hence I am not
posting the url just yet, it needs a few tweaks).
The main problem is that there does not appear to be the ability to
add a few explanatory notes of text before the questions themselves
(at lesat I do not find such functionality as a screen reader user).
The problem with S.M. is the Fieldset/Legend problem, if I put a lot
of text into the question itself, which I have to, that text is read
entirely for pretty much every choice in the radio button or check box
fieldset (for some reason it is only read for the first 3, not the
rest of them) with Jaws 14.
If I could put the explanatory text before the question, I could make
it short and sweet, making it easier for the user to check or select
their answers.
Also I am surprised that the radio button / check boxes are announced
after the text that goes with them is read.
Sample screen reader output (when navigating by arrow keys)
Q: Which screen reader do you use?
"I use Jaws for Windows"
"checkbox not checked".
....
It seems more logical to me to have it the other way round, and I am
used to that (the check box being announced before the description of
the choice it represents).

This may be confusing to screen reader users who are not
professionals, and that is the group I want to reach out to, and make
sure I get feedback from.

So if anyone has done a lot of work with S.M. would you mind
contacting me off-list with some tips?
