Carol,

Regarding your second question - the implementation of td scope and/or
axis attributes, in conjunction with title tags, and th's should
alleviate any possible confusion caused in this instance by the
repetition of link names.

Similarly the use of fieldsets as you've begun to do, and colgroups
should help with the complex tables. In this context, headers and form
element labels while serving similar functions, really are different -
refer to the link above for examples of complex form/table element
identification. As usual, only testing/observation will tell if the
information is truly accessible for the AT users.
