I'm not sure about questions for your questionnaire.
However, if you are interested in gauging the level of accessibility in
an organization or coming up with a list of items to prioritize, then
you might take a look at the "Accessibility Maturity Model" developed by
the Business Disability Forum (BDF):

The resources provided on that page are free to use and include an
"In-depth Accessibility Maturity Model Questionnaire".
