Dona,
I'm wondering if the source document might be the issue. I've had weird things like that happen when the source document was created in an older version of Excel or Word. I've also had attributes disappear if I've edited the PDF at all. In those cases, I had to add the attributes again. Generally, it has happened when the source document was from an older version of Office (or was in an older file format).
