If the CSS doesn't load, then it reverts to the browser's default.
A primary reason people turn off outlines is because they don't like
the visual appearance of the browser defaults - and this varies
greatly by browser. I don't think there's anything wrong with
customizing it - especially if it can be made visually appealing and
match the site's visual design.
WCAG doesn't address contrast for keyboard focus indicators. I believe
this is an item to hopefully be addressed in WCAG 2.1. Regardless of
what WCAG requires, users need visually distinctive focus outlines in
order to meaningfully navigate a site.'
